DESCRIPTION

Looking for a driven and passionate Case Manager for our Ryan White Program!

Case Management services serve as a hub for communication, collaboration, and support for patients working with interdisciplinary teams. Case Managers at HOPES provide Non-Medical Case Management services to patients by completing Social Determinants of Health (SDOH) screenings, developing a Care Plan, and providing referrals and related service navigation that improve patient overall health outcomes.

Case managersuse a holistic, patient centered approach to identify barriers to health outcomes. Using evidence-based and harm reduction rooted practices, Case Managers provide care coordination in partnership with patients to meet their individual goals. Case Managers work with patients through completing assessments, care planning, group facilitation, service enrollment, and continued case review with the goal of improving patient self-sustainability and improved health. Case managers work with interdisciplinary teams and community resources to address identified barriers and vulnerabilities in support of the patents identified goals through care planning and continued support as needed. The case manager is responsible for upholding department and agency standards for patient care by engaging in case consultation, interdisciplinary meetings, and continued educational opportunities.

Case Manager caseloads and populations are assigned to the case manager based on department and agency need. Department populations assigned under this position are identified as follows:

Ryan White Case Management- Eligibility, Medical Case Management - Treatment Adherence, Non-Medical Case Management, Retention in Care, Integrative Medical or Non-Medical care coordination for patients experiencing or at risk for homelessness. HCM identify barriers to care, and provide linkage or program enrollment to internal or external resources that improve patient self-sustainability and health.

POSITION REQUIREMENTS

*Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

Professional or Para-Professional experience in the case management or treatment of individuals in any of the following care coordination areas: Medical/Nonmedical, HIV+, Housing, Behavioral Health, or targeted care coordination for vulnerable populations such as pediatric, geriatric, or LGBTQA+

De-escalation skills and/or experience in de-escalating patients is required

Able to work autonomously as well as collaborative
