Job/Position DescriptionFoster Care Redesign Adm PS VIThis job posting will fill three positions.Under the direction of the OCBCT Director this position is responsible for directingCommunity Based Care implementation and operations activities, and serves asthe catchment area's liaison to the Single Source Continuum Contractor (SSCC).The position interacts routinely with DFPS staff, the SSCC, other agencies in thecommunity, community boards, judges, schools, and the general public.This position can be located at any DFPS office location with the possibility of teleworking. Essential Job FunctionsDirects, plans, implements and monitors Community Based Care catchment-level implementation and operations activities;Serves as catchment area liaison to SSCC;Builds effective relationship/partnership with SSCC;Establishes and maintains engagement of and cooperative relationships among Community Based Care stakeholders, including representatives of community, consumer, staff and public interest groups;Manages development of joint protocols and other SSCC/CPS procedures;Confers with program staff, the judiciary, advocates and other community stakeholders to identify trends and identify and resolve Community Based Care concerns;In coordination with the statewide Community Based Care Director, develops and manages catchment-specific implementation plan;Coordinates Community Based Care cultural change activities at the catchment area level;Plans, organizes, and conducts meetings and workgroups with internal and external stakeholders;Develops and coordinates catchment area implementation support team;Coordinates resolution of Community Based Care case-specific issues;Gathers, tracks and reports data as requested;Manages evaluation/Quality Improvement activities at catchment area level and attends State level Community Based Care meetings as requested;Refers requests for Community Based Care information from the media and, in coordination with Public Information Officer, plans and directs informational programs to maintain favorable stakeholder perceptions of Community Based Care;Prepares and makes presentations and conducts public meetings in support of Community Based Care;Attends work regularly in accordance with agency leave policyDiscerns and directs appropriate contract issues to contract manager. Registrations, Licensure Requirements or CertificationsKnowledge Skills AbilitiesKnowledge of the community and community stakeholders; knowledge of agency program and contract policy, procedures and regulations; skill in establishing and maintaining effective working relationships and in managing teams; skill in stakeholder engagement and developing consensus; skill in problem-solving and resolution of disputes; skill in effective oral and written communication; ability to develop and use project management processes, practices and tools; ability to gather, assemble, correlate and analyze facts; ability to travel locally within the catchment area (region) as well as outside the catchment area on an as needed basis; ability to work non-traditional hours as necessary.
